Service 06
Assistance with Life Stages and Transitions
Periods of change can create practical gaps even when a participant knows what they want. This service provides structured assistance to organise, prepare for and work through changes in everyday life.
The Service
Practical assistance through change.
This service provides structured assistance to organise, prepare for and work through changes in everyday life.
Included Support
What this service can include
Practical steps are established around the participant's transition and agreed actions.
01
Planning practical steps around changes in routine, living arrangements or other life transitions.
02
Assistance to organise appointments, information and agreed actions.
03
Building routines and organisational strategies for greater independence.
04
Supporting the participant to identify options and communicate preferences.
05
Connecting agreed actions across relevant informal, community and funded supports where within scope.
How We Approach It
Clarity without taking over decision-making.
We break a transition into practical steps, identify what the participant
can do independently, where assistance is required and who is responsible
for each action. The purpose is to improve clarity and confidence, not to
take over decision-making.
Who This May Suit
Support through everyday transitions.
Participants who need practical capacity-building assistance to navigate a change, establish routines or manage day-to-day transitions.
Quality, Safety & Accountability
Clear boundaries and agreed scope.
This service is delivered within the boundaries of the registration group
and agreed service scope. It is not represented as specialist support
coordination, legal advice, financial advice or clinical case management.
