Support Connection and Coordination
Need help understanding your NDIS plan, finding suitable providers or coordinating services across health, housing and community systems? Our Support Coordination team works with participants to build confidence, organise supports and keep the plan focused on individual goals, preferences and choice.
Care of Australia provides basic support coordination focused on understanding your needs and helping plan suitable care and supports. We will explain our connection to other services if we discuss one of our services as an option for you. You can choose Care of Australia, another provider or a combination of providers. Your choice of other providers will not affect the support coordination you receive from us. We disclose and manage conflicts of interest and can provide our policy in a format you can understand.
